Bierhoff debuted in Germany back in the late 1980s as a junior international in Uidingen, where he wasn't too bad, but failed to hold down a starting spot. The decision to move to Hamburg was the bottom of his career. After two seasons in which Bierhoff only managed to score 6 goals in 31 league games, which was unacceptable for a striker, Bierhoff moved to Borussia Mönchengladbach in the winter of 1989-90, where he still failed to score in 8 games. Bierhoff decided to join the lower level of the Austrian league and play for Salzburg in order to pursue his sporting career. Birkhoff scored 23 goals in one season with Salzburg, and he even scored 4 goals in one match.

On the strength of his goal-scoring exploits with Salzburg, Bierhoff was offered a move to small Italian club Ascoli. Birkhoff became well known at Ascoli, and his career peaked in the summer of 1995 when he joined Udinese, a mid-tier Serie A club. He scored 57 goals in 86 matches in the league, and in his final season he became the top scorer in the Italian Serie A with 27 goals, making him the center of attention in Italian soccer.

Birkhoff only started playing for big clubs later in his soccer career. After the 1998 World Cup, Birkhoff officially moved to AC Milan, at a time when the Serie A giants were having a difficult time, finishing in the middle and lower parts of the league table for the last two seasons (1996-97, 1997-98). In his first season with the club, Birkhoff helped AC Milan win the 1998-99 Serie A title. This was Bierhoff's first league title in Serie A, and he was a key figure in AC Milan's success in the league.

In 2001, Birkhoff moved to French Ligue 1 club Monaco, before returning to Serie A to play for the lesser Chievo after one season. Birkhoff's last appearance for Chievo was against Juventus in the league, where he scored a hat-trick in a 3-4 defeat. By the time he retired in 2003, he had scored a total of 103 goals in Serie A, one of the highest scoring non-Italians in the league's history.
